To apply for coverage please submit a completed Acord application along with one of the below supplemental applications and 5 year currently valued loss runs if applicable.

Help with Applications:

  • Brokers should submit the application by email to
    vos.Energy@Schinnerer.com or by fax to
    201-284-5661.
  • Customers should send the completed application to your broker. If you do not have a broker, call us at 201-284-5660 for a broker near you.

  • Have you faxed in your FCC form? Recent FCC regulations require that we obtain permission from you before we can fax you anything, including quotes. If your office has not submitted a signed form, please include one along with your application so we can respond promptly to your request.
