Named Insured under the policy.

All phone numbers for the insured and/or their designated representatives.

Date, Time and Location of the loss or damage.

If multiple buildings are insured under one policy, specify which building is damaged and the specific address listed on the policy.

Description of the loss & damage.

All coverages and applicable policy limits for the loss location including: Building, Contents, Business Income, Signs and any Additional Property Coverage.

Location where the damaged property can be inspected, if moved from the insured location.

A contact name and phone number at the loss location.
