Claims for TWIA should be submitted by our office.  Please call 800-284-4747 or 800-284-4746 and a representative from our office will collect the information needed to open your claim. If you are unable to reach a representative from our office, you can report your claim to TWIA directly at 800-788-8247. You can also submit required claim forms directly to TWIA via fax or email.

Send completed Loss Notice Claims Form by fax to 512-899-4953 or by email to TWIAlossnotices@twia.org (send completed form as an attachment to the email)
